Usage Agreement

This survey is part of a statewide study that is required by federal laws, specifically 674(a) of the Individuals with Disabilities Education Act (IDEA) and 115.77(4)(j)9 of the Wisconsin Statutes.

There is a law called the Family Educational Rights and Privacy Act (FERPA) that protects the privacy of school records. This law generally prevents schools from giving out information about students without their permission, except for certain reasons. (For supporting documentation, see the letter from the State Director of Special Education under "Survey Instruments".)

This study is one of those reasons. Because this study is required by law, former schools can use information from student's school records without their permission or their parent's permission if the student is younger than 18. All answers and documents are to be kept private.

Please have your Superintendent or Special Education Director/Pupil Services Coordinator complete the following:

The district ensures that all those involved in collecting and reporting of this survey data will keep the individual responses/documents confidential and secure, and that responses with fewer than 5 will not be shared publicly. The Superintendent or Director of Special Education gives permission to a designee, i.e. St. Norbert College Strategic Research Institute or a district designated and trained person(s) to complete a local outcomes survey on behalf of the district.

The district acknowledges that using the Indicator 14 website fulfills SPP/LPP Indicator 14 requirements. Local data for Indicator 14 will be reported via the website for:

  • Enrolled in higher education within one year of leaving high school.
  • Enrolled in higher education or competitively employed within one year of leaving high school.
  • Enrolled in higher education or in some other postsecondary education or training program; or competitively employed or in some other employment within one year of leaving high school.
  • Calculate the district's DPI Special Education Transition Incentive Grant Funding (https://dpi.wi.gov/sped/about/state-performance-plan/indicators/14-post-high-school) amount.
